1. You can fill in information on-screen and
on-line by going to this page.

When you complete your data entry, this page can be
e-mailed directly to the RAA office.

2. You may download a file in the format
of your choice to be completed and mailed.

If you choose the Microsoft Word Document
and have that program on your computer,
you will be able to enter data directly on-screen.
You will still have to print out the form and mail or fax it to RAA.
